Rules:

1. Bring in a picture of your child with the following info on the back. This is the considered the membership fee.

• Child's Name
• Address
• Phone Number
• Birthdate
• Parent's Name
• Parent's E-mail Address

Membership cards will not be given out without a picture.

2. Each month a child's picture will be drawn. That child will be "Creations Kids Klub Of the Month". A picture will be hung in the salon. (The child's name can only be drawn once per year.)

3. The Kids of the Month will receive a free haircut that month only. The haircut value is based upon age of the child.

4. The Kid of the Month will also receive a $10.00 Gift Card to Toys "R" Us.

5. The ages for membership are 0-13.

6. Each child in the membership will receive a Klub Card. With each haircut they will ge a punch in the card and on the 7th punch they will receive $2.00 off a cut.

7. The punch card is for that child only. No exceptions.

8. Parents must renew the child's membership once a year with an up to date photo and info.
